Case Manager

Title: Case Manager
Description:

POSITION SUMMARY:

Performs utilization review, discharge planning and quality monitoring functions.  Acts as liaison between patients, staff, physicians and/or outside organizations.  Utilizes the computer to enter clinical reviews, discharge planning notes and arrange disposition.  Facilitates IV to PO medication conversions.

     

EDUCATION / TRAINING / EXPERIENCE:

Minimum:

(1) Minimum one year recent utilization review and/or discharge planning experience in acute care setting.

Desired:

(1) Previous experience with managed health plans

 

LICENSES / CERTIFICATIONS:

Minimum:

(1) Current California RN license, (2) BLS-C.

 

SKILLS / KNOWLEDGE / ABILITIES:

Must be computer literate.  Must be able to follow instructions, work quickly and accurately in a fast paced environment. Analytical and grammatical skills are necessary to communicate effectively, verbally and in writing.  Organizational and multi-tasking skills are essential.  Must be fluent in spoken and written English.  Must be able to fulfill the essential functions of the position.

 

PHYSICAL JOB CLASSIFICATION:

Sedentary Work

PHYSICAL/COGNITIVE REQUIREMENTS:

Extensive telephone and laptop computer use.  Repetitive motions of wrists and hands in use of computer.

ENVIRONMENTAL CONDITIONS:

Possible exposure to biological hazards, noise, dust, heat, cold and vibrations. Majority of work is performed indoors, protected from the elements but not necessarily from temperature changes.     

 

PATIENTS SERVED:

This position is responsible for age-related physiological, emotional and cognitive needs of the following age groups of patients: Neonate, pediatric, adolescent, adult, and geriatric.  Includes, but not limited to:  Knowledge of differences in physical and psychosocial discharge planning requirements.
