Diabetes Self-Management (Outpatient)

The Diabetes Self-Management program at Doctors Medical Center is a service bringing together physicians and diabetes educators to develop a personalized plan for each patient.  This plan gives patients the resources and knowledge to manage their own diabetes throught healthy eating, physical activity, and blood sugar monitoring.  This plan gives patients the ability to problem solve and to adjust emotionally to living with diabetes.

Recognized by the American Diabetes Associtation, the program offers services to people with:

  • Newly diagnosed diabetes
  • Long standing diabetes
  • Pre-diabetes or metabolic syndrome
  • Gestational diabetes

The Diabetes Self Managment program has many ways for people to participate:

  • Basics Class -
    A series of two hour classes (once a week for three weeks) for those newly diagnosed with diabetes, or pre-diabetes, and for those who would like a refresher on their diabetes education.  Classes start month.  Physician referral and pre-registration is required.
  • Diabetes Self-Management Follow-Up Classes -
    Individual two hour classes designed to help patients continue to manage their diabetes successfully.  The classes focus on specific areas of diabetes self-management, covering such topics as; "ABCs of Diabetes and Heart Disease", "Meal Planning", "Carbs: Beyond the Basics", "Problem Solving Workshop", "Getting Started with Exercise", "Balancing Weight and Diabetes Management", and "Healthy Cooking for 1 or 2".  Physician referral and pre-registration is required.
  • Personal One-On-One Counseling -
    Sessions to assess specific educational needs, identify goals, provide information, and behavioral intervention and evaluation.  Physician referral and pre-registration is required.
  • Grocery Store Tours -
    Periodically scheduled one hour grocery store field trips.  Tours focus on evaluating the information on food and beverage labels, how to choose and use seasonal produce, analization of proper portion and carbohydrate choices, and information regarding the best and worst food items for diabetes and heart health.  Pre-registration is required, but a physician referral is not necessary for these events.
  • Diabetes Support Group -
    A free monthly meeting co-sponsored by the American Diabetes Association.  Meetings are held the second Tuesday of each month in Vale Conference Room at Doctors Medical Center (2000 Vale Road in San Pablo).  Physician referral and pre-registration is not required.
