Admitting
Our admitting office is dedicated to giving you fast, compassionate service. In order to facilitate your smooth transition into the hospital system we ask the following:
Patients that are scheduled to come into the hospital for any procedure should first contact admitting at (510) 970-5110 to get pre-registered into the computer. General information will be taken, including insurance information.
Before arriving for admission, please be sure to use the following guidelines:
Even if you pre-register you must enter the hospital through the Admitting Office. Be sure to arrive at the hospital approximately 30 minutes prior to your appointment and bring your insurance card.
Personal Items:
Please bring things that you know you will need such as: Eyeglasses, dentures, and hearing aids. You might also bring your own toothbrush and toothpaste.
Also bring the following:
- Proof of Insurance Coverage. Medicare and other insurance identification are needed to verify your eligibility for claims filed by the hospital.
- Advance Healthcare Directive. If you have one, please bring it with you. You can establish one at the hospital as well. Just ask your nurse for the forms and any assistance you may need in filling it out.
- Referring or Family Physician Information. Name, address, and telephone number.

Please do not bring:
We encourage you to leave all valuables, such as large sums of money, jewelry and credit cards at home. If your stay includes time in a critical care unit, non-essential belongings will need to be sent home during that time.
Upon Admittance
You may be asked to sign Consent for Treatment forms for diagnostic treatment, blood transfusions and surgical procedures performed in the hospital. These must be signed by you, or an individual legally authorized on your behalf.

Monday thru Friday 6:00 a.m. – 6:00 p.m.
Holdiays, Weekends, and off hours – Registration done in the ER 24hrs.
