Diabetes Self-Management Education

  • The management of diabetes continues to change. Successful diabetes management is a team effort. Working with your physician, our team creates a care plan for you with comprehensive self-management instruction and support. Our Registered Dietitian is a Certified Diabetes Educator with over 20 years experience in the field of diabetes care and is able to provide patient-focused and individualized care based on patient needs and preferences.

 

We  encourage your family to join you in learning how to make healthy food choices, developing a plan for regular physical activity, monitoring your blood sugar, and learning how and when to take your diabetes pills or insulin.

 

Whether you are taking the first steps toward better diabetes care, or just need a refresher course, we welcome you and your significant others to develop a self-management plan to meet your needs-- one based on your age, work schedule, daily activities, family demands, eating habits, and health problems.

 

Our services include:

 

Personal One-On-One Counseling
Individual coaching and medical nutrition therapy is available to persons with diabetes who need specialized carb counting and insulin adjustment, weight loss, lipid lowering, and kidney diets along with diabetes eating guidance. Physician referral and pre-registration is required. Family members are welcome.

 

Eating Well Class

This class discusses what diabetes is and how to eat. This class helps you to understand how foods and how much one consumes will affect their body. Carbohydrates, fats, and high protein foods are identified with serving sizes. The ins and outs of label reading will improve your skill and will enable you to enjoy the foods you love but in an appropriate portion.  We will discuss how to eat smart, when eating out. Physician referral and pre-registration is required. Family members are welcome. 

 

Grocery Store Tours

Periodically scheduled one hour grocery store field trips.  Tours focus on evaluating the information on food and beverage labels, how to choose and use seasonal produce, and information regarding the best and worst food items for diabetes and heart health.  Pre-registration is required, but a physician referral is not necessary for these events.

 

Diabetes Support Group

A free monthly meeting co-sponsored by the American Diabetes Association.  Meetings are held the second Tuesday of each month at 6:30 PM in the meeting room at the Doctors Medical Center Outpatient Center (100A Towne Center San Pablo).  Physician referral and pre-registration is not required.
